National Matching Program Code: #155921

Medicine Shoppe, Two RiversThe Two Rivers Medicine Shoppe Pharmacies consist of two integrated practice sites: a community-based pharmacy, and an office-based pharmacy specializing in medication therapy management, compounding and collaborative practice with an independent physician. The constant in this practice has been a commitment to patient focused pharmacy care recognized on a state and national level for innovation. Pioneering work with such quality-based program initiatives as the Wisconsin Pharmacy Quality Collaborative, HealthMapRx and Foundation Care has served to develop and sustain this care model. Recently re-engineered, the facility design/workflow has served as prototypes for the pharmacy profession. Three pharmacists and 12 pharmacy technicians provide staffing in these integrated sites. The Two Rivers Medicine Shoppe Pharmacies have been involved in resident training since 1999.

National Matching Program Code: #155915

Marshland Marshland Pharmacies, Inc. consists of three community pharmacies in Horicon, Mayville and Beaver Dam, as well as a specialty compounding pharmacy. The pharmacies have been designed to optimize the delivery of patient care services. The Horicon and Mayville sites are traditional community pharmacy practices, and the Mayville site also houses the compounding pharmacy. The Beaver Dam site is located in the Vita Park Medical Clinic, which is attached to the Beaver Dam Community Hospital. Our goal is to provide quality patient care through consultation, profile reviews, communication with health care providers, and documentation. Management provides opportunities for extensive experience in a variety of disease states, including asthma, diabetes, hypertension, and women’s health. We provide durable medical equipment and supplies, measure for and sell compression stockings and diabetic shoes, do compliance packaging for facilities, complete pharmaceutical care billings, participate in providing MTM services through WPQC and Medicare Part D, and immunize—including travel vaccines. We also serve as a community health information resource through radio or newspaper announcements and community education sessions. At our sites, residents will have the opportunity to interact with eight pharmacists with varying practice experience, including a former community pharmacy resident. Prior to becoming one of the founding practice sites for the UWCPRP, Marshland Pharmacies trained five residents since 2000.
