Application Deadline: January 15, 2009

This deadline applies to applicants who are seeking admission to the program in the following semesters:

  • Fall 2009
  • Spring 2010
  • Summer 2010

Required Application Materials:

Please carefully review all of the information on this page prior to starting your application. The complete application consists of an Online Application and a Supplemental Application. All application materials are required and must be submitted by the application deadline for the application to be considered complete. Incomplete applications will not be reviewed for admission.

I. Online Application:

Application Fee

  • Application Fee. $45.00 Nonrefundable Application Fee. Payable by credit card (Mastercard or Visa), debit or ATM card, personal check, or money order drawn on a U.S. bank.

Submit the Application Fee electronically through your Online Application (preferred).

Send checks or money orders written out to "University of Wisconsin - Madison" to:

Online Application

The Graduate School's Online Application must be completed. It includes these critical components:

  • Statement of Purpose. The statement should outline your:
    • Reasons for graduate study
    • Special interests in the research area
    • Background preparation
  • Recommendations. Three recommendations are required, two of which must be academic.

Submit the Statement of Purpose and Recommendations electronically through your Online Application (preferred).

II. Supplemental Application: (also required)

Official Test Scores

  • Official GRE Scores
  • Official TOEFL Scores. Required for applicants whose native language is not English or whose undergraduate instruction was not in English.

GRE and TOEFL scores are sent to us electronically from ETS. Use ETS institution code 1846.


Supplemental Application Materials

  • Financial Aid Statement. All applicants must submit this form, even if they are not seeking financial aid from the School of Pharmacy.
  • Resume. Include a synopsis of research projects and information on awards and honors.
  • Transcripts. Send two copies of official transcripts for all college work. Documents must be issued by the school with the official seal/stamp and an official signature. International academic records must be in the original language accompanied by an official English translation.
