Becoming a Clinical Site Instructor


Clinical Instructor Requirements

  Be licensed as a pharmacist for at least one year
•  Pharmacist license is in good standing with the Board of Pharmacy
  Practice at your site for at least 6 months
  Practice at your site for 32 hours per week.*
•  Complete a clinical instructor application form
•  Complete the clinical instructor training (held each April at the School of
     Pharmacy)
•  Complete the evaluation forms in a timely fashion.
  Be enthusiastic to take students!


Checklist for Clinical Instructors

  1. Complete a clinical instructor form and fax to 608-265-5421.
  2. Make an appointment for a site visit with Mara Kieser.
    * Site visits are done May - September

  3. Obtain your Personal Identification Number (PIN) for access to your School of Pharmacy
    (SOP) clerkship web page at www.pharmacy.wisc.edu/clerkship ?from Audrey Fish.

  4. Provide one paragraph describing your site on the SOP clerkship web page.
  5. Provide one paragraph describing student activities on the SOP clerkship web page.
  6. Provide usual hours students will be at your site on the SOP clerkship web page.
  7. Complete an action plan on the SOP clerkship web page.
  8. Attend the clerkship training program held annually in April at the SOP or view the online version at www.pharmacy.wisc.edu/clerkship.
  9. Update your site information on the SOP clerkship web page:
    www.pharmacy.wisc.edu/clerkship
  10. Update your personal information on the SOP clerkship web page.
  11. Sign up to take students on the SOP clerkship web page annually in September.
  12. Check for students assigned to your site on the SOP clerkship web page annually in February.
  13. Check for student feedback on the SOP clerkship web page annually in June.
