To assist us in covering the costs of the breakfast, lunch, and the various handouts that are provided at the event, a $15 per person registration fee is required. The registration fee should be made payable to the UW School of Pharmacy and must be mailed to:

You will receive two e-mails; one upon submission of your online form and one confirming that you are registered for the event. The latter email will not come until we have received payment for the Discovery Day.
Refunds will be given for cancellations made (by phone at 608-262-6234 or by email) on or before February 1, 2008. No refunds will be given for cancellations after that date.
